Good record keeping is
an accepted cornerstone of best practice, and has
been emphasised by CESDI and other surveys on patient
safety and avoidable harm. A review of the intrapartum
notes currently used in units in the West Midlands
have shown wide disparity and lack of standardisation,
and the same appears to apply throughout the country.
Often, intrapartum notes rely on addition of loose
pages, which means that it is impossible to know
whether a record is complete or pages are missing.
Standardisation is also important for the care of
mothers who need to be transferred, and for midwifery
and obstetric staff who move between units.
In the absence of a viable national initiative,
the Perinatal Institute is undertaking the development
of new intrapartum notes (‘Birth Notes’),
consistent with the principles applied in the Pregnancy
Notes, and the core dataset collected through our
new Maternal and Neonatal Electronic Recording System
(MANNERS).
Project History
- 2001- Project commenced
- Initial draft presented at sub –regional
meetings during 2002
- First draft printed early 2003, presented
at a multidisciplinary seminar
- May 2003 -First draft presented at annual regional
supervisors of midwives meeting
- July 2003- version 2 disseminated throughout the
region for consultation, further feedback received
- August 2003 – two field trials undertaken
- October 2003 – field trial findings presented
at regional forum – further feedback agreed
- December 2003 – final version printed.
- March 2004 – first regional training workshop
- May 2004 – birth notes presented in combination
with pregnancy notes during a national training
workshop
- December 2004 – evaluation commenced on
the use of the notes in clinical practice
- February 2005 - Version 5.1 produced in response
to evaluation process.
